I also have this issue with a my controller.  I have adjusted the AV sync  
correctly but the lag is in the controller.  The controller is about  
100-150ms behind.  Therefore, if  I put in 150ms AV sync, I can play it  
near perfect if I just listen and do not look at the screen.  However, if I  
put it on mute and try to play just be looking at the screen, I'll miss  
most of the notes.
